HRV with a pacemaker or beta blockers: what your tracker is really showing you
Heart-rate variability measures the small gaps between beats, and the reason it is interesting is that your nervous system controls those gaps. Take that control away — with a pacemaker, or with a drug that holds your rate down — and the measurement keeps producing a number that no longer means what the app thinks it means.

What HRV is actually measuring
A healthy resting heart is not a metronome. The interval between beats stretches and shortens continuously as your parasympathetic and sympathetic branches negotiate, and the size of that variation is a rough readout of how much recovery capacity you have. Higher usually means better rested; a fall from your own normal usually means stress, illness or a hard training load.
Every one of those inferences depends on one assumption: that your nervous system is the thing setting the interval.
What a pacemaker changes
A pacemaker enforces a floor. If it is set to pace at 70 beats per minute, your heart does not drop below 70 — and when it is pacing most of the time, the beat-to-beat interval is being timed by a circuit rather than negotiated by your vagus nerve. The variability that remains describes the device's timing, not your recovery.
Someone paced close to 100% of the time will typically see a very low, very stable HRV that barely moves from day to day. That is not a sign of a body in trouble. It is a sign that the measurement has been taken over by hardware, exactly as the hardware was designed to do.
What beta blockers and rate-control drugs change
Medication works differently but lands in a similar place. Beta blockers blunt the sympathetic side, lower resting heart rate and reshape variability; rate-control drugs for atrial fibrillation change the rhythm the watch is trying to read in the first place. The numbers are still measured accurately — they simply stop tracking the thing HRV is used as a proxy for.
The practical consequence: comparing your HRV to a population chart is meaningless, and comparing it to your own pre-medication history is meaningless too. What can still be worth watching is the shape of your own trend since the medication settled, and even then as a loose signal rather than a verdict.
Why recovery scores punish you for this
Almost every consumer recovery score folds HRV in as a weighted term, and several of them weight toward your weakest input. So a person with a device-set HRV is not treated neutrally — the invalid input actively drags the score down, every single day, for a reason that has nothing to do with how they slept or trained. Resting heart rate often does the same, because a pacing floor is not a resting rate.
It compounds: biological-age and cardiovascular-age estimates lean heavily on HRV, so the same person is told their heart is decades older than they are, on the strength of a number their pacemaker is producing.
What still tells you something
Plenty. Sleep duration, sleep timing and how fragmented the night was are unaffected. Breathing rate during sleep is unaffected. Skin temperature and its deviation from your baseline are unaffected — often the first hint of an infection. Steps, workouts and training load are unaffected. And your own subjective read on how you feel remains the measurement none of these devices can take.
