Fish oil and atrial fibrillation: the trade-off in the omega-3 evidence
Fish oil is among the most-taken supplements in the world, and unusually for that category it has large randomised trials behind it. Those trials found something the label rarely mentions: alongside the triglyceride benefit, several reported more atrial fibrillation in the treated group. That is not a reason for alarm. It is a reason to know, and to ask whether your capsule is earning its place.

What omega-3 reliably does
The triglyceride effect is solid and dose-dependent — substantial at prescription doses of two to four grams a day, modest at the one gram typical of a supermarket capsule. Beyond that the picture is genuinely contested. One large trial of a purified EPA formulation reported a reduction in cardiovascular events; another large trial of a different formulation found none, and the first has been complicated by an argument about whether its placebo was inert. For someone without a specific indication, taking a gram a day for general heart health, the honest summary is that the benefit on hard outcomes is smaller and less certain than the shelf implies.
The atrial fibrillation signal
Several large randomised trials and the meta-analyses pooling them report a modest but reasonably consistent increase in new atrial fibrillation among people taking omega-3, and the effect looks dose-related — the higher the dose, the clearer the signal. The absolute increase is small, and for someone taking a high dose under medical supervision for very high triglycerides it may well be outweighed. But it is a real finding from the same trials that are cited for the benefits, and some regulators have added it to labelling. A supplement with a documented adverse signal is a different proposition from one that is merely unproven.
What atrial fibrillation is, and why detection matters
It is an irregular and often fast rhythm arising in the upper chambers of the heart. Plenty of people have it without symptoms, which is the awkward part: the main clinical concern is that it raises stroke risk, and that risk is manageable once the condition is known. So the useful framing is not fear but detection. Undiagnosed atrial fibrillation is the problem; diagnosed atrial fibrillation is a condition with established management.
What a wearable can and cannot say about rhythm
Optical sensors in rings and watches read a pulse waveform, and irregularity in that waveform is detectable — several devices offer irregular-rhythm notifications built on exactly that, and some watches add a single-lead ECG. A ring’s optical signal can hint at irregularity but is not diagnostic, and it produces false positives, particularly with movement. Just as importantly, the numbers you actually see are not an arrhythmia screen: a night of unusually scattered HRV or an odd resting heart rate is not evidence of atrial fibrillation, and reading it that way will cost you sleep for no reason. And if your heart rate is set by a pacemaker or by rate-controlling medication, none of these patterns mean what they would otherwise. A diagnosis requires an ECG.
What to actually do with this
If you take a prescribed high-dose omega-3 for elevated triglycerides, this is a conversation to have with whoever prescribed it, not a reason to stop on your own — the trade-off may be entirely correct in your case, and stopping a cardiovascular medication unilaterally is its own risk. If you take a gram a day because it seemed generally healthy, notice that you are accepting a small documented signal in exchange for a benefit that is weaker than commonly assumed at that dose, and that eating fish has different and better evidence than capsules do.
